There is no cost for this benefit. Supervisor Application as an Adobe PDF File Supervisor Application as a 2003 Word Document Supervisor Application as a 2007 Word Document Supervisor Application as a Open Source Document Technician Certification: Members may train their own employees as biofeedback technicians and certify them with NTCB for a fee of $65 per year under the condition these technicians remain under supervision by you or a person assigned by you to supervise them. Optional certification titles that you may request are Certified Stress Management Technician, Certified Pain Management Technician or Certified Alternative Healthcare Practitioner. NTCB Technician Application As a Word Document NTCB Technician Application as an Adobe PDF File Technician Malpractice Insurance: Technicians supervised by you may become members of the Biofeedback Association of North America without being certified, and may obtain $2/10 Million dollar malpractice and $ 2/3 Million dollar general liability insurance plus $100 K for damages to your premises caused by the technician for a fee of $235 a year. Technician Malpractice Insurance Credit Card Merchant Accounts: Under our group plan you can obtain a credit card merchant account and a secure, completely confidential gateway for only $10 a month plus less than a 2% credit card fee. Members who use this service report a savings of almost 50% on their total credit card merchant account fees. Apply for a Quotation Website and Banner Creation: MBACB has a contractual agreement with Qhiron Corporation to provide the following services to MBACB members at these reduced rates: Banner Design - Sundara Designs will design a banner for your website to your satisfaction for $50 per banner. This optional benefit is normally priced $250 per banner. URL Registration - URL Registration - IPX Services will register your domain anonymously through Secret Registrations for $25 a year. You will own your own domain, but your name will not be released except when required by law to do so. Note: pornographic material on your web site is the primary cause of legal intervention by the courts. For this reason, IPX Services will not provide any services for any pornographic or other illegal activities. This service also includes protection for your domain name, including protection from hacking and hijacking. Website protection usually costs from $5.00 to $10.00 per month. This service is included in the price of your domain.
